- Incidental Paper Records (All paper documents, notes, messages, forms and memos)
- Stored Records (Records beyond their retention period or that have been converted to electronic health records “EHR”)
- Computers (Pressing “delete” or even reformatting the hard drive DOES NOT erase the data.)
- CDs, Smart Phones, Thumb Drives
